With fledgling democracies on either side of a country whose medieval mullahs are repressing a population that had been used to Western culture, the "great event" may not be the one that Ahmanedijab expects.
Then again, it might be a tactical nuke up his pooper.

Actually pretty small in terms of kilotons. The biggest concern is a ship launched Scud with a tact nuke off the US Northeast Coast.
It doesn't have to be very accurate. It can be a lobbing arch up to 100 km or so.
There would be little loss of life on the ground due to any radiation. The concern is the electro-magnetic pulse (EMP) generated from the blast would flow in the electric grid. The big hypothetical danger is frying the hardward/software electical power switching and load leveling systems.
It would be a long period before power could be restored. Longer than emergency generators have gas to run. This could cause major riots and breakdown of social structure and government.
Not good.
Enough of a threat I would make DAMN sure I knew where OBL's/AQ's Naval fleet of 15 junk cargo ships were at all times. If they approached within range of the US, I would be ready to blow them out of the water.
Remember the Mullahs are interested in all things EMP, have nuke tech courtesy of the Chinese, A.Q. Kahn and the NORKS, missile tech from them too,and have been test launching sea launched Scuds in the Caspian Sea. Mind you these test Scuds seem to destruct in midair and have been chalked up to failures. Depends on what your trying to achieve.
Even though the Mad Mullahs are Shia, they will partner with OBL/AQ, who are Sunni, when pragmatic to gang up against the Big and Little Satan.

AQ's "fleet" is a real problem and the difficulty in tracking them. That's a lotta water out there. And they're not really sure they know how many ships AQ has.
Just for the record (nitpicking, I know) but bin Laden subscribes to a far stricter Qutbism.
